Creating Safe Pathways Back to Family

The Connected Families Foundation operates a Residential Reunification Service for children and young people in out-of-home care. Our residential program offers a safe, supportive, and structured home-like environment staffed by skilled youth workers and therapeutic professionals who provide daily care, emotional support, and developmental guidance.

Unlike traditional residential care, our model is built around family reunification. We provide intensive therapeutic support, strengthen family relationships, and work closely with parents and Child Safety to create a safe, sustainable pathway for children to return home where possible.
